POSITION SUMMARY

The Social Service Director is responsible to plan, organize, develop, and direct the overall operation of the Social Services Department. This must be done in accordance with current federal, state, and local standards, guidelines and regulations, our established policies, and procedures, and as directed by the Administrator. The Social Services Director interacts with the residents, families, visitors, other facility staff, and community agencies during the admission and discharge process the Director interacts with residents, families, visitors, other facility staff. This position is responsible for patient assessments, care planning and supervising additional social services staff. Assists in development of policies regarding participation in facility planning for health and welfare services. Provides input to strategic decisions as related to social services areas of responsibility. Proactively plans and facilitates de-scalation protocol with support of interdisciplinary team and can resolve escalated situation within facility.

ESSENTIAL RESPONSIBILITIES AND DUTIES

Facilitates and Coordinates the Admission Process and Discharge Planning

  • Accepts admission referrals and obtains the required information.
  • Gives tours of facility to prospective residents/families. Distribute information about the Rehabilitation Center.
  • Makes referrals as needed for pre-admission screening and pre-admission physician visits. • Obtains pre- admission screening approvals from appropriate counties.
  • Assists residents/responsible parties with completing required admission paperwork, forwards information to Corporate Office on a timely basis.
  • Maintains resident records by entering admission summary, quarterly notes, interaction with resident, etc. into medical record per facility policy.
  • Assists residents/responsible parties with applications for financial assistance from appropriate government agencies as necessary.
  • Develops, maintains and documents ongoing discharge plans for each resident.
  • Completes the Social Services discharge summary in coordination with the interdisciplinary team.
  • Make referrals to appropriate home health agencies as requested by resident/responsible parties.
  • Assists with obtaining needed home health equipment.
  • Maintains knowledge of community resources and utilizes these appropriately at time of discharge planning.

Directs the Overall Operations of the Social Services Department

  • Assume the authority, responsibility and accountability of directing the social service department
  • Complete necessary evaluations for other social services personnel.
  • Participate in facility surveys (inspections) made by authorized government agencies.
  • Delegate administrative authority, responsibility and accountability to other social services personnel as deemed necessary to perform their assigned duties.
  • Attend department head meetings.

Communicates and Interacts with Other Staff, Families, Residents. Community Agencies and Visitors

  • Assists resident/families with adjustment to facility placement.
  • Accepts placement referrals from community agencies or individuals.
  • Acts as liaison between residents/families and the facility staff for concerns, questions, and in particular, personal needs.
  • Visits 1:1 with residents to assess psychosocial needs and to provide reassurance and support.
  • Informs residents/families of the right to make advanced directives and decisions about treatment as well as other rights.
  • Supportive to residents/families in the grieving process.
  • Informs residents/responsible parties of change in roommate.
  • Encourages family involvement and support.
  • Provides support, encouragement and information to residents that have psychosocial needs or problems.
  • Communicates to appropriate facility staff the psychosocial needs and problems of each resident.
  • Makes referrals to community individuals or agencies to meet psychosocial need

Participates in the Interdisciplinary Patient Care Planning Team

  • Completes a Patient Health Questionnaire (PHq-9) and Brief Interview for Mental Status (BIMs) per facility policy.
  • Completes a social history and assessment of each resident upon admission.
  • Documents follow-up at initial conference time and quarterly thereafter.
  • Completes assigned sections of MDS, CAA’s and summaries.
  • Participates in development of the care plan coordinate and facilitate Patient Care Conferences.
  • Identify, update and implement care plan approaches assigned to Social Services closely assessing psychosocial and behavioral needs.
  • Develops resident groups identified as facility need.
  • Facilitate Family Council as appropriate

Maintains Healthy Relationships and Uses Professional Conduct with Residents, Families, Visitors, Supervisors and Co-workers

  • Is courteous and cooperative with residents, families, visitors, supervisors and co-workers.
  • Follows dress code, wears name tag, and limits personal phone calls.
  • Is courteous and cooperative with residents, families, visitors, supervisors and co-workers.
  • Maintains resident and personnel confidentiality at all times. Understands the need for completeprivacy of all health care data.
  • Abides by and promotes the Continuous Quality Improvement Program of Monarch Healthcare Management.

FACILITY COMPLIANCE

  • Promotes a highly positive workplace centered on strong customer service, inclusivity and ensuring staff feel welcome and supported
  • Maintains departmental records of each resident with original referral, admission papers, current insurance and financial information
  • Participates in inspections and audits of department as needed to maintain safety and sanitary conditions. • Identifies areas of problem/needs through surveys and communicates these at QAPI committee meetings.
  • Educates staff, family and residents on the Resident’s Bill of Rights and the Vulnerable Adult Act.
  • Implement and educates to the Abuse Prohibition Plan as necessary.
  • Has clear understanding and knowledge base of State and Federal Department of Health certification regulations, particularly those relating to Social Services.
  • Attends educational programs on Continuous Quality Improvement, customer satisfaction and other such programs.
  • Is knowledgeable of and abides by the importance of the Residents’ Bill of Rights.
  • Ability and willingness to work some evenings and weekends as directed by supervisor.

QUALIFICATIONS

Education and Experience

  • Must recognize and understand the psychosocial and emotional needs of residents.
  • Must have requires strong skills in communication, organization, management, and leadership.
  • Must be able to perform essential functions of the position with or without reasonable accommodation.
  • Must qualify for employment after a criminal background study, per guidelines of the Minnesota Department of Human Services.

Preferred

  • A Minnesota license to practice social work.
  • Experience in healthcare, skilled nursing facility or assisted living community
