Social Services Director Less than 120 beds
The Social Service Director is responsible to plan, organize, develop, and direct the overall operation of the Social Services Department. This must be done in accordance with current federal, state, and local standards, guidelines and regulations, our established policies, and procedures, and as directed by the Administrator. The Social Services Director interacts with the residents, families, visitors, other facility staff, and community agencies during the admission and discharge process the Director interacts with residents, families, visitors, other facility staff. This position is responsible for patient assessments, care planning and supervising additional social services staff. Assists in development of policies regarding participation in facility planning for health and welfare services. Provides input to strategic decisions as related to social services areas of responsibility. Proactively plans and facilitates de-scalation protocol with support of interdisciplinary team and can resolve escalated situation within facility.
Admission & Discharge Coordination
Manages all aspects of resident admissions, including referrals, screenings, tours, and required documentation.
Assists residents/families with financial aid applications and discharge planning, including home health referrals and equipment needs.
Social Services Department Management
Directs and oversees the operations of the Social Services Department.
Delegates responsibilities, completes evaluations, and participates in facility inspections and administrative meetings.
Resident & Family Support
Assists residents and families with facility adjustment, personal concerns, and psychosocial needs.
Acts as liaison between residents/families and staff, promoting communication and family involvement.
Interdisciplinary Care Planning
Conducts psychosocial assessments, care planning, and documentation (MDS, CAA’s, summaries).
Participates in care conferences and develops group programs and family council initiatives.
Professional Conduct & Relationships
Maintains professionalism, confidentiality, and respectful communication with all parties.
Follows facility policies, dress code, and supports Continuous Quality Improvement initiatives.
Regulatory Compliance & Education
Ensures compliance with state and federal regulations, Resident’s Bill of Rights, and abuse prevention policies.
Maintains records, participates in audits, educates staff/residents, and attends training on quality improvement and customer service.
Qualifications
Must recognize and understand the psychosocial and emotional needs of residents.
Must have requires strong skills in communication, organization, management, and leadership.
A Minnesota license to practice social work.
Experience in healthcare, skilled nursing facility or assisted living community
