Nurse Case Manager
Horizon Public Health is seeking applicants for a Full Time Registered Nurse to provide case management services out of our Alexandria office. A Case Manager provides care coordination, including developing, implementing and evaluating client plan of care, coordinating care using available resources and maintaining contact with care givers. Minimum qualifications include a Baccalaureate degree in Nursing and licensed in MN with at least one year of home and community-based experience. Exceptions may apply for ADN applicants with at least two years of home and community-based experience. Prior experience in long term care also beneficial. Successful applicant must pass a criminal background check and a pre-employment drug test. Must have valid MN driver’s license and reliable, insured transportation. This is a Grade 13 MNA/AFSCME Council #65 union position, Step 1 $33.72/hour Monday-Friday daytime hours.
